If you ever have an emergency, please make it clear to our staff, and they will immediately do what is necessary.

Please arrive 10 minutes early to allow time for check-in procedures (15 minutes if you are a new patient).

Kindly give 24 hours’ notice to cancel or change appointments.
|
|
Please bring insurance information and identification to each office visit.
|
|
A parent or legal guardian must accompany children under 18 years of age.
|
|
Please bring to every visit, a current list of all your prescriptions, over-the-counter medications, supplements and remedies.
|
|
Patients will be charged for no-show appointments or appointments that are cancelled with less than 24 hours notice.
|
|
Please allow 24 hours for prescription refills to be processed during weekdays.
|
|
Diagnosis and treatment plans can not be appropriately made over the phone, E-mail, or fax. Therefore, we can not call in prescritions unless you have been seen by our professional staff. |
